As a medical practice, we have patient paperwork and other documents in Adobe Acrobat Portable Document Format (PDF). In order to improve the viewing of these files, please download the latest free version of Adobe Reader. We do understand that not all PDF documents are currently considered accessible. Due to this, we can gladly accommodate your paperwork needs also in-person during your patient appointment.

People using screen-reading devices generally are unable to read documents directly in PDF format, unless they have an accessibility plug-in installed on their system along with the Adobe Acrobat Reader. We do ask that you please utilize the plug-in that is available at no charge from Adobe. This Adobe online tool will convert PDF files to HTML on request.
